to provide holistic and coordinated care to participants through the pairing of a self-selected Family Doctor<\/li>\n<li>To achieve &#8220;early prevention, early identification and early treatment&#8221; and reduce disease complications<\/li>\n<\/ul>\n<p>[\/vc_column_text][vc_empty_space][\/vc_column][\/vc_row][vc_row][vc_column][vc_empty_space][vc_column_text]<\/p>\n<h2>Services Scope<\/h2>\n<p><span class=\"intro-l\"><span style=\"color: #ff9900; font-size: 20px;\"><b>Cardiovascular disease risk factors screening and management<\/b><\/span> (formerly known as &#8220;Chronic Disease Co-care Scheme&#8221;) &#8211;<\/span><\/p>\n<ul class=\"intro-l\">\n<li>To provide screening of diabetes mellitus (DM) or hypertension (HT)\u00a0and blood lipid testing, allowing for a comprehensive assessment of &#8220;three highs&#8221;<\/li>\n<li>Upon diagnosis of prediabetes, DM, HT or hyperlipidaemia, the participants are entitled to subsidised consultation visits for long-term management under their Family Doctors<\/li>\n<\/ul>\n<p><span class=\"intro-l\"><span style=\"color: #ff9900; font-size: 20px;\"><b>Chronic hepatitis B screening and management<\/b><\/span> (formerly known as &#8220;Hepatitis B Co-care Scheme&#8221;) &#8211;<\/span><\/p>\n<ul class=\"intro-l\">\n<li>To provide risk-based hepatitis B screening and management<\/li>\n<li>Participants who are diagnosed with chronic hepatitis B are entitled to subsidised consultation visits, with arrangements the same as those of the cardiovascular disease risk factors screening and management<\/li>\n<\/ul>\n<p>[\/vc_column_text][vc_empty_space][\/vc_column][\/vc_row][vc_row][vc_column][vc_empty_space][vc_column_text]<\/p>\n<h2>The Co-care Network provides a structured framework for preventive care and chronic disease management, organised into two main categories:<\/h2>\n<p>[\/vc_column_text][vc_empty_space][vc_btn title=&#8221;One-off preventive activities&#8221;
