Service Scope
Kowloon City District Health Centre Express provide three levels of health prevention service:
1. Health promotion, education and counselling service
2. Health risk factor assessment and disease screening
3. Chronic disease management
Citizens with HKID are eligible to register as DHC Express members and attend free health assessment service, followed by other levels of service depending on heath needs.
First level of prevention service:
All members are welcomed to join series of free heath promotion activities, with topics include:
· Healthy diet, weight management, physical activities
· Smoking and drinking related harms
· Maintain good mental health
· Pain management
· Drug management
· Disease prevention
· Carers support
· Other seasonal health topics
The Care Coordinator will conduct individual consultation and follow up when necessary.
Second level of prevention service
Free health risk factor assessment:
Health risk factor assessment will be conducted by Care Coordinator or other allied healthcare professional, with basic physical check-up, review of living style, daily mobility, mental health assessment, etc. According to the result of assessment, the Care Coordinator will formulate individualized health management plan towards risk factor identified and suggest to attend different health education activities in order to maintain good health. Chronic disease screening may further be offered whenever necessary.
Chronic disease screening
Diabetes, Hypertension: In case of risk of Diabetes or Hypertension is identified after the assessment, eligible members can pair with family doctors and join the “Chronic Disease Co-Care Pilot Scheme”.
Low back pain, OA Knee: In case of risk of Low back pain, OA Knee is identified after the assessment, eligible members will be offered for continuous care and health monitoring.
Eligible members who are newly diagnosed with diabetes or hypertension could enjoy medical consultation services and individual healthcare services.
Third level of prevention service
For those with confirmed diagnosis of Low back pain or OA Knee, specific chronic disease management programme will be offered for continuous care and health monitoring.
Members with unknown history of diabetes mellitus or hypertension are eligible to join the “Chronic Disease Co-Care Pilot Scheme” and pair family doctor.
